What Does a Rheumatologist Do?
A rheumatologist diagnoses and treats diseases that affect the joints, muscles, bones, and immune system. These doctors care for arthritis, autoimmune conditions, inflammatory diseases, osteoporosis-related concerns, and many disorders that cause persistent pain, swelling, stiffness, or fatigue. Unlike an orthopedic surgeon, a rheumatologist primarily provides nonsurgical treatment and manages conditions that often require long-term monitoring.
What conditions does a rheumatologist treat?
Rheumatologists treat more than 100 different conditions, including common forms of arthritis and less common systemic diseases. Their work focuses on conditions caused by inflammation, abnormal immune activity, degeneration, infection, metabolic changes, or a combination of these factors. Some diseases affect one joint, while others involve the skin, eyes, lungs, kidneys, blood vessels, nerves, or other organs.
Common conditions treated by rheumatologists include rheumatoid arthritis, osteoarthritis, gout, lupus, psoriatic arthritis, ankylosing spondylitis, juvenile idiopathic arthritis, Sjögren's disease, scleroderma, myositis, vasculitis, and polymyalgia rheumatica. A rheumatologist may also manage osteoporosis and other disorders that weaken bones, particularly when the condition is severe, occurs at an unusually young age, or does not respond to initial treatment. Fibromyalgia is sometimes evaluated or managed in rheumatology practices, although it is not an autoimmune disease and treatment may involve primary care, pain medicine, physical therapy, sleep care, and behavioral health professionals.
How does a rheumatologist diagnose a condition?
Diagnosis begins with a detailed medical history and physical examination. The rheumatologist asks when symptoms began, which joints or body areas are affected, whether symptoms change during the day, and whether factors such as activity, rest, illness, or injury influence them. The doctor also reviews other symptoms, including fevers, rashes, dry eyes, dry mouth, mouth sores, changes in weight, weakness, numbness, and changes in bowel or urinary function.
The physical examination helps the rheumatologist distinguish inflammation from other causes of pain. Depending on the situation, testing may include measures of inflammation, blood counts, kidney and liver function, uric acid, antibodies associated with autoimmune diseases, and tests for conditions that can mimic inflammatory arthritis.

Ultrasound can identify fluid, inflammation, tendon problems, and certain changes that are not visible on a standard examination. In some cases, the rheumatologist removes a small amount of fluid from a swollen joint using a needle. This procedure, called joint aspiration or arthrocentesis, can relieve pressure and help identify crystals associated with gout or pseudogout, infection, blood, or inflammatory changes. Testing joint fluid is especially important when a sudden swollen joint could be caused by an infection, because that problem requires prompt treatment.

Why do rheumatologists monitor patients over time?
Many rheumatic diseases are chronic, which means care continues after symptoms improve. How is a rheumatologist different from other doctors?
A rheumatologist is an internal medicine physician who completes additional training in diagnosing and treating rheumatic and autoimmune diseases. A primary care doctor may evaluate joint pain, order initial tests, treat straightforward conditions, and refer a patient when symptoms are persistent, unusual, severe, or difficult to explain. The rheumatologist provides specialized assessment when inflammatory or systemic disease is a concern.
Orthopedic surgeons focus on the structure and mechanical function of the musculoskeletal system and perform operations when surgery is appropriate. They may treat fractures, torn ligaments, severe joint damage, and conditions that require reconstruction or replacement. Rheumatologists generally treat the medical causes of joint and tissue problems without surgery, although they may refer a patient to an orthopedic surgeon when structural damage requires an operation.
Other specialists may also be involved. A physiatrist focuses on rehabilitation and physical function, while a pain specialist addresses complex pain conditions. A dermatologist may treat skin psoriasis and help manage psoriatic arthritis in coordination with rheumatology. The appropriate specialist depends on the suspected cause, the tissues involved, and the patient's broader health needs.
When should someone see a rheumatologist?
A referral is worth discussing with a primary care clinician when joint pain, swelling, or stiffness persists without a clear explanation. Referral is especially relevant when symptoms affect several joints, cause prolonged morning stiffness, return repeatedly, or interfere with work, sleep, walking, or hand use. A primary care clinician may also refer a patient after finding certain patterns on examination or abnormal laboratory results.
Symptoms involving more than the joints can also point to a condition that needs rheumatology evaluation. Examples include unexplained fevers, unusual fatigue, recurring mouth sores, sensitivity to sunlight, a persistent rash, dry eyes or mouth, color changes in the fingers with cold exposure, muscle weakness, or unexplained changes in kidney or blood tests. These symptoms do not prove that an autoimmune disease is present, but they provide useful reasons for further assessment.
A suddenly hot, swollen, and severely painful joint requires prompt medical attention. Infection, crystal arthritis, and other urgent problems can resemble an autoimmune flare. A person with chest pain, severe shortness of breath, sudden weakness, confusion, or other emergency symptoms should seek immediate emergency care rather than waiting for a routine rheumatology appointment.
What should patients expect from rheumatology care?
Rheumatology care is based on shared decisions and repeated reassessment. The patient and doctor discuss the diagnosis, treatment goals, expected benefits, possible side effects, monitoring needs, and alternatives. A treatment plan should also account for pregnancy or plans for pregnancy, vaccinations, infections, other prescriptions, allergies, and access to follow-up care.
Patients can make appointments more useful by describing specific symptoms and their effect on daily life. Notes about the location and duration of pain, morning stiffness, swelling, rashes, medication use, and side effects can reveal patterns that are difficult to recall from memory.
